Aging Consultation Services
Our Aging Consultation is a comprehensive, one-time session with a Geriatric Care Manager designed to help you understand your current situation and plan for what lies ahead.
During this consultation, we:
- Assess your living environment, health needs, and support system
- Identify current challenges and potential risks
- Provide individualized recommendations tailored to your goals and preferences
- Offer referrals to trusted professionals and home care providers
This service is ideal for individuals and families who want expert guidance and a clear path forward.
Ongoing Case Management
Our Ongoing Case Management provides hands-on, continuous support for older adults and their families. As needs change, our care managers remain actively involved to ensure services are coordinated and effective.
Services may include:
- Coordination and accompaniment to medical appointments
- Communication with healthcare providers and family members
- Monitoring in-home care and other services
- Assistance with transitions (hospital discharge or rehabilitation)
- Support in reducing isolation through social and community connections
